• Photo copy of birth certificate or other proof of date of birth
• Contact Form (fill out online)
• Uniform Purchase/Order Form
• Photo (submit as email attachment or mail in) - for id passcard
• $95 (payable to Lee Soccer - see below)

We will determine which teams we are able to field for the spring 2012 season no later than January 20 based on player commitments in-hand. In the case of insufficient registrations in a division we will return the registration fee.
